We collect information you provide directly, most notably through our Pilot Application form: your name, job title, organization, work email address, phone number, number of communities you operate, the primary care types you offer, and any additional notes you choose to share. You do not need to provide any of this to browse the Site, only to submit an application. We also automatically receive standard technical information from your browser, such as device and browser type, through our website host, Framer, and through our video hosting provider when you watch a demo video.
We use the information you submit to evaluate and respond to Pilot Program applications, to communicate with you about WiseGuides, and to understand how prospective partners engage with our demo content. We do not use your information to make automated decisions that produce legal or similarly significant effects, and we do not sell your personal information.
We share information only as needed to operate the Site and respond to your inquiry: with Framer, Inc., which hosts this website, and with our video hosting provider, which streams the demo video and reports basic engagement, such as which video was watched and for how long, back to us. We do not share your information with third parties for their own marketing purposes. We may disclose information if required by law or to protect our legal rights.
The Site is built and hosted on Framer, which may set essential cookies needed for the Site to function. Our demo video is delivered through an embedded third-party player, which may use its own cookies or similar technologies to play the video and report basic engagement back to us.
